Isn't 1st degree the norm when it's pre-meditated / pre-planned with 2nd degree more the "crime of passion" kind of situation?
If the reports are correct with respect to the guy just going there to confront the doctor and then shit escalated to the stabbing, wouldn't that be 2nd degree?
I could be way off base on the criteria for 1st vs. 2nd too.

I'd say bringing a knife qualifies as planned and deliberate. I'm going to guess it probably passes the criminal harassment test as well, unless this is the first time confronting the doctor.

Could the argument be made that the knife was purely for intimidation purposes and he had no intent on killing until the taunting escalated things (assuming that's actually what happened)?

^ yes, but as rage2 mentioned that would fall under criminal harrassment and it would mean the murder was 1st degree.

I think there is a lot of evidence that we aren't privy to that led to a first degree murder charge, which can't be laid without consent of the Crown after review of the evidence.
Obviously there are people with far more knowledge and ability who make those calls, but I'm not one of them and can't offer much insight about that stuff!
