I think part of the problem is the errors go completely undetected and therefore the persons making them do not get replaced or demoted. When I worked for OLG, I reported a problem regarding the size of the Lottario rollover jackpot. The person at the other end kept trying to "teach" me how the game worked and how rollovers worked. It took a large effort on my part to both convince them I knew what I was talking about and an error had been made.
