Comes with the box, and all the cables and adapters, manuals too. Pretty much mint condition.
Never overclocked, so can't say how far it will go, but with a good cooler you should get PE speeds and maybe more!
This one's got Samsung memory chips (GDDR3); 1.6ns memory access time, which should perform at PE memory speeds if you overclock it.
