Assuming that they don't have a healthcare system over there, wouldn't heathier people be paying less premiums like good drivers do for auto insurance?

Therefore, I would think that insurance companies lose money. Yeah, they would get money from fines for overweight people, but most companies won't allow that to happen.

Just my thought.
