If I'm just casually driving I don't bother with ear plugs. For any 'spirited' (above 4000rpm) driving, I have to - without them, after 4000 I can phsyically feel something in my ear trying to rip itself out (not ringing, but actual phsyical pain, like somebody's ripping a part of my ear out). With the plugs it's a-oh-kay though....however, woe be to those who're driving around me :D
