I have.

When I went you needed to get a special permit from a travel agency in addition to a Chinese visa. I was on my own in Lhasa, but used a driver from the agency that provided the permit to take me though the rest of Tibet and to Everest base camp. I didn't really have time to mess around with local transport.

Not too familiar on coming from China aside from permits mentioned. In Nepal the general option is an ~8 day trip driving 2-3 days each way in shared SUVs with about 3 days between lhasa and one other town. That the chinese government approves. Where all the other tourists are. Tour agency gets your Tibet visas for you, Nepal visa on arrival in Kathmandu. Pretty easy.

That said, a couple germans i met had gotten an agency to do all the paperwork as if hired, but were able to go alone on a bicycle trip over. Probably had to pay a lot of bribes along the way

Almost any type of trip should be possible, standard style trips through tour groups require minimal planning though.

I've heard mainlanders are willing to pay upwards of $1 million for a Tibetan Mastiff
