Hey so i'm renovating my place (1960s era), and i've got a question about how to replace my heat registers. I'm hoping someone has had some experience or has any suggestions.

The problem is that the current registers are old and beat up, they sit up against the wall, and are are not a standard size. See pics for more details.

Anyone have any ideas on what to do? Can i get custom ones made? Anyone know who can do them?

That register won't fit, the lip facing the wall would need to be cut. And beyond that, there would be no room for the baseboard.

I have the same issue and used 2x12 and 2x10 registers, trimmed off the lip closest to the wall, and beveled the bottom of the baseboard out. Looks sweet and modern for now. Eventually I'll make some ones that sit flush with the hardwood floor.
