Well, considering it's a dually... I'm guessing at one point he did have a fifth wheel hitch. They are easy to remove and he might have needed it out of the truck to haul something in the bed.
FYI 5th wheel hitches are easily removable... most people do that when they need to use the bed for something.

At least it doesn't look TOTALLY out of place - given that they *are* installed for purposes already described. (Even if he never pulled a trailer)
