How to transfer music/videos from old ipod to new?

Hey guys,

Recently got a new ipod touch to replace my old one.

I want to extract all my music and videos from my old ipod to my new ipod.

Problem is, my itunes library isn't the same anymore because i switched computers long ago and most of this music was extracted from CDs long long ago.

I found a way online of looking into the hidden folders and extracting the music, but when I do that...all my songs end up with whacky titles instead of original song name/titles.

What software or method would you recommend to do this transfer properly, with full track titles and everything?
