I've seen the Bluetooth receivers, but am wondering if it it would be easier and cheaper to use one that has a built in speaker instead of transmitting to FM.

How old school are we talking about? Do you have an aux port?
I have one of these, which does more than what you're asking for as it plays music as well.

The cord is kinda short, so I have it plugged into the center armrest and the puck is under my e-brake level. No issues with sound quality and I've never been told I'm hard to hear on the other end.
Food for thought: If you're considering one of those visor ones, think about how you're gonna keep the unit powered, or how much you're using it and will need to charge its battery.

Why not just get a new Cheapo deck from Amazon?
It would be like $35 for one with bluetooth function, it'll probably have USB as well.
Go to Canadian tire and buy the $15 harness kit and depending on the car you have you might not even need a dash kit.
More than what you need, but i'm sure you wouldn't mind having USB. once you start using it; you wouldn't even know how you lived without it originally

It's a 2003 Civic and no ports at all.
I thought about a new deck, but for various reasons, that's out the question.
I had this one in a previous vehicle:

It worked quite well, connects to the phone via bluetooth and handles calls as well as plays music through FM transmission. Has a USB port on the side for charging as well as line in and line out 3mm jack for non-bluetooth media devices.
I think I still have mine in a box somewhere that I would be happy to sell you for $20 if interested.
If you were thinking of using one with built in speaker why not just use the speakerphone function on your phone and use a vent or windshield mount to keep it in place?
there are a ton of bluetooth hands free that just clip on your visor and plug in to USB...
Check out Jabra, or Blueant, those are 2 good names.

And if by "car doesnt have any ports" you mean cigarette lighter spots, you can wire one in super easy, 12v+ground, maybe fused for good measure.

I just bought a new car that has AUX, USB, and Bluetooth. I start the car, and hands free calling is automatically initiated. With the new car, hands free calling is one of those features that I've found, "I can't do without". Kind of the same with the gf riding shot gun in the new car with heated seats. Her recreation of pleasure leads me to believe that a quick drive with heated seats on (while playing Thunderstruck, takes me out of the tedious component of foreplay.
So, what's the best way to do the same in my old Civic that yet again, has no data ports?

Just get this, $180 with the parts to keep your steering wheel controls, if you have those ($120 if you don't). No CD player means it's brainless to install and nobody uses CDs in their car, even me, and I'm a guy that buys 20-40 CDs a year.

As long as you have bluetooth on your phone turned on when you get in the car, the bluetooth FM transmitter I had (and still have somewhere) did exactly that. Start the car, it connects and can play your music as well as take/make calls.
