I've very frequently had both of my doctors and my father's (who is South African, and a family friend, my father is godfather to his daughter), bring this subject up, and I've witnessed several incidents of patients raising their voices to the point of yelling, and one storming out and knocking magazine racks over, after being given the good news that they will no longer be getting their painkillers, or at least a fraction of what they've been used to. Again, just a WAG (wild ass guess), but if I was betting $, this is what I'd put it on based on recent experiences as mentioned.
