I'd definitely look at a self-guided multi-night bike tour, if time permits. Wife and I did one with https://cicloposse.com/ through Pienza, Montalcino, Montepulciano ending in Sienna, in 2013 and it's easily my favourite memory from our two weeks in italy. We'd booked with a more SEO-optimized, english speaking firm but they were just re-selling cicloposse services.

We ate at amazing restaurants, and really experienced the country in a way I've struggled to replicate in most other travel. Strong recommend.

Thanks everyone! Appreciate the suggestions and will probably end up spending some more time there after hearing and looking into all the great destinations. If anyone has specific restaurants or hotels to stay at, it would be awesome to hear. Cheers

I don't know the tourist locations so well to go to but I know where you can get a pretty authentic experience. There is a hotel, which is more like a bed and breakfast in the Town where my family is from. Link is below. It's right on the edge of the town and they took the old church along with some other properties. It is absolutely beautiful, the Town dates back about 600 years or so. If you're lucky enough to be there on April 28 that's the Festa della Madonna delle Grazie. That's a huge festival in the Town, but I am betting that the hotels in the area are booked for that time already. You won't get much more authentic than that. If you can be there at the time I would try really hard to. Again, most of the younger folks in the Town will speak some english, certainly at the hotel.

Oddly enough found a video covering the entire Town.

    Rome
Florence - fashion outlet mall to keep wife happy
    Lake Como
Southern Italy - Salerno, Paestum ( Ancient Greek ruins)
Venice ( don’t need more than a day/ 2 here) but it’s nice to experience if you’re in Italy.

Agree with Venice. Spent a day there and that was it for me.
If you and your wife is interested in shopping and going to Lake Como, Lugano is less than an hour north and has a massive outlet mall as well.

Isn't the Ducati factory in Bologna? Or one of the other Italian art/bike manufacturers? I would tour the shit out of that!
