I dont doubt that that the level of effort can vary to a certain degree but that just means that some have to work a little harder than others to achieve the same goals. BMR however is a function of size, weight and age. Those that are obese have a higher BMR of course because they weight more at the beginning. I think its a silly metric however and shouldn't be really considered when planning to lose weight over the more important TDEE. The best way and often the cause for failure (IMO) is using weight measurements + honest calorie counting to establish a true baseline. Then subtract a reasonable number from that as your target intake. Often people under estimate their calories and add too aggressive deficit. They see some results but are miserable because they are quite literally starving themselves.
Hormones, i agree, are a factor however I feel like they are more to do with how you FEEL as opposed to simply losing the weight. Again, making them miserable and setting themselves up for failure. Weight loss must be a lifestyle change and a gradual one at that but people pump their bodies with this garbage then go back to drinking 2L of coke per day and eating 200% their current TDEE once they lost the 20lbs they wanted.
Yes, CICO is over simplifying a complex problem but its the place to start and effective for vast majority of the population.

I used to sell this drug many years ago when it was Victoza. Might have been 2011 or so. Side effects were not desirable from what i can remember. Terrible nausea, bloating, feeling full all the time. Patients took it because it did a great job of controlling their blood sugar levels. The weight loss seemed to be a byproduct that may not have been worth it. That was then. Maybe the new once a week version is better now. Going for lunch with one of the reps who sells it this week. I'll ask her for some details on it.
